Requests for Funding

DCLRN

Funding Requests :

Mid-July, families requiring respite care will be sent the annual Request for Funding forms which need to be filled out and sent back to our office immediately.

Respite requests for the traditional $200 stipend per family is given on a first come-first serve basis. When you receive your form in the mail, don't delay!

We have 24 initial slots to fill; 8 families who care for a child birth to 18 years of age; 8 slots for families with someone at home with a disability between 19-55; and 8 slots 55 and older for seniors with a disabiltiy. Lifespan means that no one, no matter what age or with what disability, should be rejected to receive respite care if it is needed. If you should submit your form for respite after all spots are filled, families will receive a notice saying that they are on a wait-list.

Through grants, donations, etc. or when another family opens up some of their funding, then we are able to remove someone from the wait list and offer them a stipend. Families will be notified when this occurs.

If you have questions about funding, please contact program director Karla Jensen at
(920) 356-9870 or email our office at dclrn@powerweb.net.
